Description: Based on observation and interview, the provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.

Evidence:
1.The provider's zoning permit requires a conditional use permit for any family day home that employs persons from outside the home.
2. During interview, Caregiver #1 reported she does not have a conditional use permit from zoning to authorize the provider to employ a caregiver who is not a resident of the home.
3. During the inspection, the licensing inspector observed one caregiver that did not reside in the home. The caregiver confirmed that she did not live there.
4. The provider provided documentation to show that a request was made for a conditional use permit on 03/12/2020, and the zoning meeting would be scheduled for 04/23/2020.

Plan of Correction: Application was submitted, waiting on the certificate.
