Comments:

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 06/08/2020 and concluded on 06/12/2020. The provider was contacted by telephone to initiate the inspection. There were 2 children, totaling 5 points, and 1 provider present during the inspection. The inspector reviewed 2 children's records and 2 caregiver records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-111-120-B
Description:	Based on record review and discussion, the provider failed to ensure that the record for each caregiver shall contain all of the elements as required by the standards. Evidence: The record for Caregiver #2 did not contain the following information: verification of age and documentation that two or more references as to character and reputation as well as competency were checked before employment.

Standard #:	22VAC40-111-60-B
Description:	Based on record review and discussion, the provider failed to ensure that each child's record contained all of the elements as required by the standards. Evidence: 1. The record for Child #1 did not contain the following: the policy number of the child's medical insurance (a name of insurance provider was provided) and results of the health examination for the child as required by the standards. 2. The record for Child #2 did not contain the following: immunization records for the child as required by the standards and results of the health examination for the child as required by the standards.

Standard #:	22VAC40-191-60-C-2
Description:	Based on record review and discussion, the provider failed to obtain a central registry finding within 30 days of employment. Evidence: The record for Caregiver #2 had a documented hire date of 11/17/2019; the date on the central registry findings was 02/19/2020.
