1. Child #1, #2 ,#3 and #4 did not have a signed Policy for the Administration of Medication for the Family Day Home.
2. Child #1, #2, #3 and #4 did not have a signed review of the Family Day Home's Liability Insurance Declaration form based on 63.2-1809.1 of the Code of Virginia.Plan of Correction: Missing documentation will be completed and signed and placed in children's records.
