Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on June 5, 2020 and concluded on June 10, 2020. The provider was contacted by telephone to initiate the inspection. There were three children present and one staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-111-120-B
Description:	Based on review of two records, the family day home provider did not obtain all rerequired documentation for one caregiver's record. Evidence: 1. The record of caregiver #1, hired 02/19/2020, contained documentation of two references , both dated 03/04/2020. The references were not obtained prior to employment as required. 2. The provider acknowledged during the remote inspection that the references were obtained after employment for caregiver #1.
Plan of Correction:	Per the provider: I will collect all paperwork and references before I hire assistants. m
Standard #:	22VAC40-191-60-B
Description:	Based on review of caregiver records, the provider did not obtain the person's completed sworn statement or affirmation before employment or providing volunteer services. Evidence: Caregiver #1, hired 02/19/2020, had documentation of a completed sworn statement dated 02/20/2020, exceeding the first day of employment.
Plan of Correction:	Per the provider: My assistant quit on me and i needed a new assistant right away but I will make sure to have them complete the sworn statement at the time of hire.
Standard #:	22VAC40-191-60-C-2
Description:	Based on review of caregiver record and interview, the provider did not obtain a central registry finding within 30 days of employment. Evidence: 1. Caregiver #1, hired 02/19/2020, did not have documentation of a central registry finding on record exceeding 30 days from employment. 2. The provider stated she did not have a central registry finding for caregiver #1 who had been working.
Plan of Correction:	Per the provider: I sent the central registry request off in April. I will check on the status. Moving ahead I will get the central registry within 30 days from hire. The assistant is not working right now because of the virus and low numbers in the daycare.
Standard #:	63.2-1720.1-B-2
Description:	Based on review of two records and interview, the family day home provider did not obtain satisfactory results of the finger-print based national criminal background check for one caregiver prior to employment. Evidence: 1. The record for Caregiver #1 (hired 02/19/2020) contained satisfactory results of the finger-print based national criminal background check dated 04/08/2020. Satisfactory results of a finger-print based national criminal background is required to be obtained prior to being hired. 2. The family day home provider verified the information reviewed during the remote inspection was accurate.
Plan of Correction:	Per the provider: I needed a new assistant right away and didn't realize i had to get the fingerprints first. In the future I will get the fingerprints before I hire someone.
