Based on interviews the complaint is determined to have a preponderance of evidence. A violation notice was completed regarding the violations found during the complaint inspection.

Evidence:
1. Staff # 1 stated that on 3/2/2020 a fly swatter was used to spank a 3 year old to get him to go to the bathroom.Plan of Correction: Provider stated that the parent encouraged her to do this. Provider stated that it was a tap on the outside of the child's clothing. The provider will not use any form of forbidden actions on children in care.
