This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 07/13/2020 and concluded on 07/17/2020. A complaint incident was received by the department regarding allegations in the areas of 22VAC40-111-30, 22VAC40-111-70, 22VAC40-111-140, 22VAC40-111-500, 22VAC40-111-560, 22VAC40-111-580, 22VAC40-111-650, 22VAC40-111-840, 22VAC40-111-870, 22VAC40-111-890, 22VAC40-111-910 and 22VAC40-111-920. The provider was contacted by telephone to conduct the investigation. The inspector emailed the provider a list of documentation required to complete the investigation.
