1. Child #6 (age: 19 months) and Child #1's (age: 3 years old) records did not contain documentation of proof of identity.
2. Child #1's record did not contain documentation of the annual review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review was dated 1/31/2019.Plan of Correction: I will have parents bring missing documents tomorrow. I will get the missing signature during pick-up today.
