Comments:
On July 1, 2020 an initial virtual inspection was conducted. Due to the COVID 19 pandemic the inspector was not present in the home but was able to conduct virtual inspection via mobile video application guided by the provider. The home, emergency supplies and procedures were reviewed. During the virtual inspection, there was an adequate amount of child toys and equipment available for operation]. Provider and household member documentation was also inspected Child and staff forms to be used were also reviewed. Violations observed during this inspection are indicated in the violation notice.

Description: Based on review of required documentation for residents over the age of 18 in the home it was determined that all 2 household members did not complete out of state CPS Checks for a state loved in within the last 5 years.

Evidence: During virtual inspection provider did not have out of state CPS checks available.

Plan of Correction: The proper paperwork will be filled out.
