Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated and concluded on Tuesday, August 19. 2020. The provider was contacted by telephone to initiate the virtual inspection. There were 4 children present and 1 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of children's record and interview, the caregiver did not ensure to obtain documentation of a physical examination within 30 days after the first day of attendance.

Evidence: In Child#1's record (start date 11/10/19) there was no documentation of a physical found. Interview with caregiver confirmed physical documentation was not available for review during this inspection.

Plan of Correction: The caregiver will request physical documentation from the parent and include documentation in record.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's record and interview, the caregiver did not ensure to obtain all information for children's records.

Evidence:In Child#1's and Child#2's records, the name, address, and telephone number for one emergency contact was documented. Interview with caregiver confirmed the documentation available for review during the inspection.

Plan of Correction: The caregiver will contact parents to obtain information for a second emergency contact both the children and update in each record for review.
