Evidence: In Child#1's record (start date 11/10/19) there was no documentation of a physical found. Interview with caregiver confirmed physical documentation was not available for review during this inspection.Plan of Correction: The caregiver will request physical documentation from the parent and include documentation in record.

Evidence:In Child#1's and Child#2's records, the name, address, and telephone number for one emergency contact was documented. Interview with caregiver confirmed the documentation available for review during the inspection.Plan of Correction: The caregiver will contact parents to obtain information for a second emergency contact both the children and update in each record for review.
