Technical Assistance:
Discussed with the provider that one child's emergency medication expired in April 2020 and the parent should reach out to the doctor for new medication or documentation to extend the expiration of the medication.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 8/20/20. The provider was contacted by telephone to initiate the inspection. There were seven children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, the provider's record, and one household member's record. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the facility.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 8/27/20. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-700-A
Description: Based on review of one medication and interview with the provider, the provider did not have written authorization from the parent to administer medication to a child in care. Evidence: An emergency medication was onsite for Child B, but the provider did not have written authorization from the parent to administer the medication.

Plan of Correction: Parent signed form at pick up to administer medication. Parent also filled out medication authorization form at pick up and has a doctor appointment next week to have the doctor complete their portion.

Standard #: 22VAC40-111-730-D
Description: Based on interview with the provider, one child's medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection the provider retrieved an emergency medication from Child B's bookbag that was in a cubby on a top shelf with no method for locking.

Plan of Correction: Already purchased a medication security bag.

Standard #: 22VAC40-111-70-A
Description: Based on review of two children's records, parents were not provided in writing all required information before the child's first day of attendance. Evidence: 1. The record for Child A, with a start date of 1/29/18 did not contain documentation of whether or not the provider carries liability insurance. 2. The record for Child B, with a start date of 6/4/19, did not contain the signed policy for the administration of medication.

Plan of Correction: Parents signed the form at pickup.

Standard #: 22VAC40-111-220-B
Description: Based on review of documentation and interview with the provider, the provider did not get retrained within three years of receiving Medication Administration Training (MAT). Evidence: The most recent MAT certificate on record for the provider is dated 11/7/2015 and the provider confirmed she has not retaken the class.

Plan of Correction: Completed the online part of the course today and am looking for a class to sign up for the in-person part.
