This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A (monitoring/renewal) inspection was initiated on August 21, 2020 and concluded on August 21, 2020. The Provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff for a total of 24 points. The Inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed children?s records and staff records submitted by the facility to ensure documentation was complete. A virtual inspection was conducted to observe the program and physical environment. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility or the program. Standard #: 22VAC40-111-200-C Description: Based on interview and record review, it was determined the Provider had not maintained dated documentation of the orientation that included signatures of the Provider and assistant . Plan of Correction: I have recorded the information and will keep in her file.
