Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 9/18/2020. The provider was contacted by telephone to initiate the inspection. There were 6 children present on the date of inspection and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, the provider's records, and 3 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Friday, 9/25/2020. Violations:
Standard #: 22VAC40-111-30-A
Description: Based on record review and provider interview, the provider did not ensure compliance with the terms of the current license issued by the department.

Evidence:
1. The provider is currently licensed to care for children ages 3 years through 5 years 11 months.
2. On 9/15/2020, 3 of 7 children in care were under age 3. Child #4 is 2 years 8 months, Child #6 is 2 years 11 months, and Child #7 is 2 years 8 months.

Description: Based on record review and provider interview, the provider did not ensure that a Sworn Disclosure Statement was completed for all adult household members.

Evidence: Adult Household Member #3 does not have a completed Sworn Disclosure Statement on file.
