Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A non-mandated focused inspection was initiated on 10/02/2020 and concluded on 10/05/2020. The provider was contacted by telephone to initiate the inspection. There were 6 children and one provider present during the inspection. The inspector reviewed 2 children's records, 1 household member record and 1 assistant record during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the provider.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and discussion, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. The record for Child #1 did not contain a physical examination. The documented start date for Child #1 was 08/03/2020.
2. The record for Child #2 did not contain a physical examination. The documented start date for Child #2 was 08/24/2020.

Plan of Correction: Will notify the parents and request a copy right away.

Standard #: 22VAC40-111-110-B
Description: Based on record review and discussion, the provider failed to ensure that the immunization record was signed by a licensed physician, the physician's designee or an official of a local health department.

Evidence:
1. The immunization record for Child #1 was not signed by a licensed physician, the physician's designee or an official of a local health department.
2. The immunization record for Child #2 was not signed by a licensed physician, the physician's designee or an official of a local health department.

Plan of Correction: Will notify parents and request a new copy.
