Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 10/7/20. The provider was contacted by telephone to initiate the inspection. There was 1 daycare child and 4 household children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed one child?s record and the provider's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards and one violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description: Based on record review, one child's record did not contain documentation that proof of identity was verified within 7 days of enrollment. Evidence: The record for Child A, with a start date of 4/9/18, did not contain documentation of verifying the child's identity.

Plan of Correction: I requested this from the child's mother during the inspection.
