Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 10/06/20 and concluded on (10/08/20. The directowas contacted by telephone to initiate the inspection. There were 7 children present totaling 16 points and 1 caregiver. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a record review the licensee did not ensure each child had documentation of a physical examination within 30 days after the first day of attendance.
Evidence:
Child #1, enrolled 07/14/18 did not have documentation of having a physical examination.

Plan of Correction: The licensee contacted the child's parent and will have a copy today at pick-up.

Standard #: 22VAC40-111-60-A
Description: Based on a record review the licensee did not maintain an up-to-date record for each child.
Evidence:
Child #1, enrolled on 09/24/2018 did not have documentation of an annual review of the child's emergency contact information.

Plan of Correction: The contact information has been reviewed and the licensee will have the parent sign-off at the bottom of the enrollment form.

Standard #: 22VAC40-111-70-A
Description: Based on a record review the licensee did not ensure parents were provided in writing all required information before the child's first day of attendance.
Evidence: Child #3, enrolled 10/15/19 had a medication delcaration form that was signed by the parent, however the form was not competed with the medication intentions of the licensed home.

Plan of Correction: The licensee will fill out a medication form and ask the parent to re-sign.
