Violations:
Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative documents, it was determined that parents have not been provided with all required operating information in writing.
Evidence:
The provider has not included the sample general daily schedule in the written enrollment information given to parents.

Plan of Correction: I usually send a weekly activities schedule but I'm not doing that while the children are currently engaged in online learning. I will create a sample schedule and add it to the written enrollment information for parents.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training documents, it was determined that there is no documentation showing that the provider has completed orientation for the substitute provider.
Evidence:
The provider has not documented the orientation training for the current substitute provider.

Plan of Correction: The substitute is fully aware of all aspects of the child care program. The documentation will be completed.

Standard #: 22VAC40-111-320-G
Description: Based on provider interviews, it was determined that the wood burning fireplace and associated chimney have not been inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
The provide has not obtained a fireplace inspection.

Plan of Correction: I will have the wood burning fireplace inspected and forward the documentation to my Licensing Inspector.
