Comments:
A Complaint investigation was begun on 9/21/2020 in response to an allegation being received on our office. Based on multiple interviews, the preponderance of evidence does not support the allegation.
The Complaint is not Valid.
Other violations cited were found during the course of the investigation and were not part of the original complaint.

Description: Based on the provider's statement and review of documentation the General Permission slip signed by the parent does not include all destinations to which the children have been taken.
Evidence:
The form available does not reflect another home where the children have been taken.

Plan of Correction: I will update the form and have the parents sign it.
