Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on November 23, 2020 and concluded on November 24, 2020. The provider was contacted by telephone to initiate the inspection. There was 1 children present and 1 provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 provider record and 2 household member records submitted by the facility to ensure documentation was complete. The inspection also included a virtual portion related to items reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-111-100-A
Description:	Based on record review of 2 children's records and provider interviews, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for child #2, (first date of attendance 5-4-2020) did not include a physical available for review during the inspection.
Plan of Correction:	Child #2 medical physical was not available at the time of the inspection. I will receive a copy upon his return to care the week of November 30, 2020.
Standard #:	22VAC40-191-40-D-4-a
Description:	Based on a review of 1 provider record and 2 household member records and the provider interviews, the provider failed to obtain from a person living in the home of licensed family day home provider a criminal history record check, within 30 days of a person beginning to reside in the home. Evidence: The record for household member #1 did not include a criminal records check/ fingerprint check. Provider confirmed the household member moved into the residence in April 2020 and they had not yet obtained the fingerprint check.
Plan of Correction:	During the noted months, I made multiple attempts during June/July/August and September to make an appointment through the Field print website. I was unable to make the correct type of appointment due to not having the correct field code that was provided to Ms. Sylvia Bennette from DSS. After Sylvia Bennette made several attempts via e-mail during the last 3 months to get the correct field code information from DSS, the correct field code was provided earlier in November to Mrs. Sylvia Bennette?s e-mail. On Saturday November 21, 2020, I went and completed the scan of my fingerprints.
