Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 11/23/2020 and concluded on 11/24/2020. The provider was contacted by telephone to initiate the inspection. There were 0 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.
The information gathered during the inspection Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on review of three children's records, the provider did not maintain each child's file as required. Evidence: the parent completed one set of paperwork for two siblings - each enrolled child needs to have an individual file. Child 1 and Child 2 did not have documentation of the name, address and telephone number of each custodial parent's place of employment, the address and phone number of the child's physician, the name, address, and phone number for a second emergency contact, and the name and policy number of the child's medical insurance.

Plan of Correction: Provider will request the missing information from the parent.

Standard #: 63.2-1721.1-A
Description: Based on review of records and provider interview, the provider failed to have a completed repeat search of the central registry (CPS check) within five years from the date of the most recent CPS check.
Evidence: The central registry check on file was dated 04/10/2015. The caregiver stated in a phone interview that a repeat search of the central registry had not been mailed.

Plan of Correction: LI emailed the CPS check from to provider.
