This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 11/30/2020 and concluded on 12/14/2020. The provider was contacted by telephone to initiate the inspection. There were 2 children, ages 23 months-3 years, totaling 5 points and 1 provider present during the inspection. The inspector reviewed 2 children's records, one provider and one household member records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-60-B Description: Based on record review and discussion, the provider failed to ensure that a child's record shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child #1 did not contain the following information: the names and addresses of previously attended child day care and the immunization records for the child as required.
2. The record for Child #2 did not contain the following information: the names and addresses of previously attended child day care.Plan of Correction: Will get the missing information from the parents.
Standard #: 22VAC40-111-70-A Description: Based on record review and discussion, the provider failed to provide to parents in writing with the information as required by the standards.
Evidence: The record for Child #2 did not contain information regarding whether or not there is liability insurance and the provision of the family day home's emergency preparedness and response plan.Plan of Correction: The forms will be given to the parent to complete.
