Technical Assistance:
The provider is informed that documentation that background checks have been requested or received must be provided to this office with in ten days of receipt of the notification.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on and concluded on December 11, 2020. The provider was contacted by telephone to initiate the virtual, remote inspection. There were five children present ranging in age from 12 months to 2 years old with the provider and one other caregiver. The children present totaled 13 points. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed five children?s and four household/caregiver records during the virtual, remote inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview the provider failed to obtain a physical examination report for each child as required.
Evidence:
1 - The record for child 1 does not contain a physical examination report.
a. Child 1 has a documented first day of attendance as August 21, 2019.
2 - The provider confirmed that there is not a physical examination report on file for child 1.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview the provider failed to obtain an immunization record for child 1 prior to the first day of attendance.
Evidence:
1 - Child 1 was present for care during the inspection however there is not an immunization record available for review in the child's record.
a. Child 1 has a documented first day of attendance as August 21. 2019.
2 - The provider confirmed that there was an immunization records available for review for child 1.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review and interview the provider failed to ensure that each caregiver obtains an updated tuberculosis screening every two years as required.
Evidence:
1 - The most recent tuberculosis screening available for the provider is dated April 25, 2016.
2- The most recent tuberculosis screening available for household member 1 is dated April 13, 2018.
3 - The most recent tuberculosis screen available for caregiver 1 is dated April 5, 2018.
4 - The provider stated that the tuberculosis screenings had not been updated.
