Technical Assistance:
The Licensing Inspector has reviwed with the provider COVID-19 Essential Guidance for Child Care Programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and completed on December 7th, 2020. The provider was contacted by telephone to initiate the inspection. There were 5 children present with 1 caregiver. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 1 caregiver's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1's record does not have documentation of a physical examination. The child's first day of attendance was 07/18/2019.

Plan of Correction: I will request the form from the parent.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, the provider failed to obtain and retain all required documentation for children's records.

Evidence:
The written authorization for emergency medical care (should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds) was not available for Child #2.

Plan of Correction: I will have parent sign form and place in child's record file.

Standard #: 22VAC40-111-80-A
Description: Based on a review of children's records, the provider failed to obtain within seven business days of the child's first day of attendance at the family day home, verification of identity and age of the child.

Evidence:
Child #2's first day of attendance was 11/08/2020, but the child's record has no documentation or verification of identification.

Plan of Correction: I will get the verification from the parent.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia by the first day of attendance.

Evidence:
Child #1's record does not have documentation of immunizations. Child's first day of attendance was 07/18/2019.

Plan of Correction: I will contact child's parent for immunization form.
