Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced initial inspection was conducted today from 9:00am to 11:00am. There were no children in care with the provider at the time of inspection. The family day home?s policies, procedures, supplies, and equipment was inspected. There were numerous age and stage appropriate materials for the requested age range. Inspection of the two cribs on site was made and determined to the meet the U.S. Consumer Product Safety Commission (CPSC) standards. The provider is current in CPR and First Aid training. The home was found to be clean and organized with adequate space available for the requested capacity of 6 children.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Tuesday, 1/19/2021.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain all required supplies.

Evidence: The first aid kit was missing tweezers and 1 of 2 required triangular bandages.

Plan of Correction: Your comment is noted and complied, and a complete set of first aid was supplied at our facility.

Standard #: 22VAC40-111-770
Description: Based on observation and interview with the provider, it was determined that a working battery-operated flashlight and a working battery-operated weather band radio was not available in the family day home.

Evidence: The provider stated she did not have a battery-operated weather band radio at the time of the initial inspection. The flashlight available was not working at the time of the initial inspection.

Plan of Correction: Your comment is noted and complied, and a complete set of battery-operated flashlight and a working battery operated weather band radio is supplied at our center.

Standard #: 22VAC40-111-330-A
Description: Based on interview with the provider, it was determined that a landline telephone was not available.

Evidence: A landline telephone was not available at the family day home during the time of the initial inspection. The provider stated she does not have a landline telephone.

Plan of Correction: Your comment is noted, and we already have a contract with T-Mobil and a request was made and we'll have the land line active in a few days.

Standard #: 22VAC40-111-430-A
Description: Based on observation, children under two years of age have access to two sets of stairs.

Evidence: The stairs coming from the main floor down to the programming space and the stairs leading up to the deck from the backyard did not have a gate in place that prevents access to children.

Plan of Correction: Your comment is noted and complied. We installed gates in the two area you commented.
