On 1/11/21 a virtual renewal inspection was completed. Due to the COVID 19 pandemic the inspector was not on site. This inspection was completed via mobile video application as well as email and phone correspondence. During inspection there were 5 children present, observed playing freely and under the direct supervision of the provider. The facility and emergency supplies and procedures were inspection. Staff and Household members records were reviewed. Violation are indicated in the violation notice.

Standard #: 22VAC40-111-50-A Description: Based on review of documentation Vendor did not have a record of children in attendance each day.
Evidence: During inspection, vendor did not have attendance available for children in care that day.Plan of Correction: I was writing it in the calendar provided and ran out of space I will keep attendance on paper.
Standard #: 22VAC40-111-60-B Description: Based on review it was determined that all required information was not maintained.
Evidence: During inspection Child A was missing verification of identification.
Child B did not have the action required for allergy stated by provider.Plan of Correction: I will get all the information
Standard #: 22VAC40-111-180-A Description: Based on review, it was determined that tuberculosis screenings were not maintain every 2 years as required.
Evidence: During inspection provider's was expired.Plan of Correction: I will get it done as soon as possible.
