This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on February 3, 2021 and concluded on February 4, 2021. The provider was contacted by telephone to initiate the inspection. There were nine children present and two staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-230-H Description: Based on review of two caregivers' records and interview, the vendor did not ensure one caregiver complete the department's health and safety update course.
Evidence:
The record of caregiver #2 contained documentation of the caregivers' completion of the required Virginia Preservice training on 11/19/2017. The record also contained documentation of the Virginia Annual Health and Safety Update Training dated 06/28/2020. Subsidy venders and staff were required to have a certificate of completion for this training on-file by March 1, 2020. Caregiver #2 did not complete the training by March 1, 2020 as required.Plan of Correction: Per the provider: I will make note of the time frames for training and make sure they are completed on time.
