Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 2/10/2021 and concluded on 2/12/2021. The provider was contacted by telephone to initiate the inspection. There were no children or staff present with the provider. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

If you have questions regarding this inspection you may contact the Licensing Inspector, Laura Brindle, at laura.brindle@dss.virginia.gov or 540-905-2062.

Violations:
Standard #: 22VAC40-665-230-H
Description: Based on interview with the provider, the provider did not ensure that all caregivers completed the health and safety update course by March 1, 2020 as required. Evidence: The provider stated that she had not yet completed the course.

Plan of Correction: I will complete the training as soon as possible.
