This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 02/16/2021 and concluded on 02/24/2021. The provider was contacted by telephone to initiate the inspection. There were 6 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-100-A Description: Based on record reviews, the provider failed to obtain documentation of a physical examination within 30 days after the first day of attendance.
Evidence:
1. The inspector reviewed two children's records. Child record #1 did not contain documentation of a physical exam and child's first day of attendance was 12/14/2020. Child record #2 did not contain documentation of a physical exam and child's first day of attendance was 12/08/2020.Plan of Correction: The provider has requested a copy of each child's physical from their parents. The parents have agreed to provide a copy of their child's physical by the end of next week.
