This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on February 25, 2021 and concluded on February 26, 2021. The Provider was contacted by telephone to initiate the inspection. There were 12 children present and 4 staff for a total of 38 points. The Inspector emailed the Provider a list of items required to conduct the inspection. The Inspector reviewed children?s records and staff records submitted by the facility via virtual inspection to ensure documentation was complete. The provider has all up to date background clearances and TB Screenings for household members and caregivers.
Information gathered during the inspection determined a non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility or the program. Please call with questions 703-479-4679

Standard #: 22VAC40-111-60-B Description: Based on record review, it was determined the provider did not maintain all children's records as 1 child file did not contain all required information.
Evidence;
1. Child 2's file did not contain 2 of 2 emergency contacts as it only contained 1 emergency contact. The same file did not contain an immunization record and has been enrolled since January 4, 2021.Plan of Correction: I will talk with the family and I read the medical form incorrect and will obtain. It will be completed and added to the file.
