Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 03/04/21 and concluded on 3/5/21. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined non-compliances with the standards, and violations were documented on the violation notice issued to the facility. Standard #:	22VAC40-111-140-B-3
Description:	Based on the review of two staff records, the provider failed to take CPR training in the last two years,. Evidence; Based on review of 2 staff records the CPR certificate expired on 10/5/2020, the leniency guidelines makes the expiration date 1/5/2021 for the provider .
Plan of Correction:	Due to the pandemic we did not go to any classes in the last year. We will look for a place to take a class.
Standard #:	22VAC40-111-180-A
Description:	Based on the review of two staff files the staff failed to get subsequent TB tests done after two years. Evidence: The review of two staff records show that TB tests were done on 7/25/2018 .
Plan of Correction:	We will get the subsequent TB test done, we have had it done because of the pandemic. By next week will both be vaccinated and will get new TB screenings done.
