Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on March 3, 2021 and concluded on March 5, 2021. The provider was contacted by telephone to initiate the inspection. There were five children present totaling 11 points which included two resident children . The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children's records submitted by the family day home to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-111-100-A
Description:	Based on record review, in one of two children's records reviewed, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The provider confirmed that there was no documentation of physical examination in the record for Child #2 (start date 9/8/2020).
Plan of Correction:	I will re-request the physical exam for Child #2 and then add to the child's file.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, in two of two children's records reviewed, the provider did not ensure that each child's record contained all required information. Evidence: The provider confirmed that the records for Child #1 and Child #2 did not have the policy number of the children's medical insurance.
Plan of Correction:	I will request insurance policy information from parents and then add to the children's files.
