Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on March 4, 2021 and concluded on March 5, 2021. The Provider or was contacted by telephone to initiate the inspection. There were 6 children present and 2 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined a non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility or the program.

Violations:
Standard #: 22VAC40-665-230-H
Description: Based on discussion, it was determined a Caregiver did not complete the Annual 3 hours in updated Health and Safety training.
Evidence;
1. the Vendor had completed the updated training but stated she did not realize the assistant needed it as well.

Plan of Correction: I will have her sign up for the class as she completed 16 hours in training but we were unaware of this class.
