Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 03/02/2021 and concluded on 03/03/2021. The provider was contacted by telephone to initiate the inspection. There were 10 children present (27 points) with the provider and one caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 4 caregiver/household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on record review, shelter-in-place procedures were not practiced a minimum of twice per year. Evidence: 1. A shelter in place drill was documented on 08/06/2020. A second shelter in place drill was not documented for the year 2020.
Plan of Correction:	We will do shelter in place drills twice a year.
Standard #:	22VAC40-111-110-B
Description:	Based on record review, 1 of 2 children's physicals did not contain all required information. Evidence: 1. The physical examination in the record for Child #1 was not signed by a licensed physician.
Plan of Correction:	I will ask the mom to send the papers.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, 1 of 2 children's records did not contain all required information. Evidence: 1. The record for Child #1 did not contain the policy number of the child's medical insurance.
Plan of Correction:	Corrected during inspection.
Standard #:	22VAC40-111-320-G
Description:	Based on record review, the provider did not ensure that the wood burning fireplace and chimney in the family day home was inspected annually. Evidence: Documentation provided by the provider indicated the last inspection and cleaning of the fireplace was completed on 4/10/2019.

Description:	Based on record review, fingerprints were not obtained within 30 days of a person residing in the home becoming 18 years old. Evidence: 1. Household member #1 turned 18 on 10/29/2020. Results of a fingerprint based background check were received on 01/25/2021, more than 30 days after household member #1 turned 18.
