63.2 Licensure and Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on February 26, 2021 and concluded on March 5, 2021. The provider was contacted by telephone at 1:30 p.m. to initiate the inspection. A virtual inspection was conducted beginning at 2:00 p.m. There were nine children present and two staff. An eight month old was observed sleeping in a crib.
Older children were observed napping during the inspection. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed four children?s records, and three staff records submitted by the facility and by video conferencing to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of documentation, Tuberculosis (TB) screenings or tests were not updated every two years as required. Evidence: Staff #1's most recent TB test was dated 5/22/2018. Staff #2's most recent TB test was dated 11/23/2018.

Plan of Correction: Both staff will complete TB screenings/test within 14 days.

Standard #: 63.2-1720.1-B-4
Description: Based on review of documentation, it was determined that the provider did not obtain documentation regarding out of state central registry background checks within 30 days of employment for employees whom resided outside of Virginia in the last five years as required. Evidence: Staff #3, start date 1/6/2021, indicated out of state residence in the past five years in North Carolina and New Hampshire on the Sworn Statement. There was no documentation of central registry background checks for North Carolina or New Hampshire.

Plan of Correction: Both have now been requested and in the future, all timeframes for out of state background checks will be met for new staff.
