Comments:
This Inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 03/15/2021 and concluded on 03/15/2021. The provider was contacted by phone to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 of staff/household records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-750-A
Description: Based on a review of records, one record does not have a current authorization for application of a topical ointment.
Evidence:
Child #1's record includes a topical ointment authorization that is dated 11/2019 (expired). The authorization may not exceed one year from the date of parent signature.

Plan of Correction: I will have the parent fill out a new form.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, the plan has not been reviewed and update annually.
Evidence:
The plan is dated 6/6/2019 for the last review.

Plan of Correction: The plan was reviewed and dated during the inspection.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records, the record for one child does not include written acknowledgement of all required policies and procedures.

Child #1's record does not include written acknowledgement for the Provisions of the Emergency Preparedness Plan.
