This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on March 16, 2021 and concluded on March 17, 2021. The provider was contacted by telephone to initiate the inspection. There were six children present and one staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-230-H Description: Based on review of caregiver records, the provider did not ensure caregivers who work directly with children obtain, in addition to preservice and orientation training annually attended at least 16 hours of training, to include the department's health and safety update course.
Evidence:
Caregiver #1 did not have documentation of completing the annual health and safety training within the past 12 months. The most recent documentation of health and safety updated training for caregiver #1 was dated 01/25/2020. The health and safety updated training is required to be complete every 12 months.Plan of Correction: Per the provider: I will complete the health and safety training every year.
