63.2 Licensure and Registration Procedures
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 3/16/2021 and concluded on 3/16/2021. The provider was contacted by telephone to initiate the inspection and the inspection was completed via FaceTime. There were 8 children present (27 points) and 2 caregivers. The Inspector reviewed 2 children?s records and 4 caregiver/household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 22VAC40-111-830-B Description: Based on record review, shelter-in-place procedures were not practiced a minimum of twice per year.
1. A shelter-in-place drill was practiced in August 2020. A second shelter-in-place drill was not practiced in 2020.
Plan of Correction: We will do a shelter-in-place drill this week. We will make sure we do two drills every year.
Standard #: 22VAC40-111-60-B Description: Based on record review, 1 of 2 children's records did not contain all required information.
1. The record for Child #1 did not contain the policy number of the child's medical insurance.
Plan of Correction: I will ask the parent for the information today.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review and interview, a person living in the home did not complete a sworn statement or affirmation when the person in the home became 18 years old.
1. Household Member #1 became 18 years old on 12/18/2020. The record for Household Member #1 did not contain a sworn statement.
2. The provider acknowledged that Household Member #1 has not completed a sworn statement.
