Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 03/18/2021. The provider was contacted by telephone to initiate the inspection. There were (5) children present, (17) points, with (2) caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (2) non-compliances with applicable standards or law and were documented on the violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on review and interview, 1 of 2 children files indicated that the Provider did not obtain documentation that a child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) before the child may attend the Family Day Home (FDH).

Evidence -
1. On the date of inspection (03/18/2021), documentation of an immunization page for Child #5 was not available for review.
2. Child #5 was in attendance on the date of inspection.
3. The Provider stated (during an interview on the date of inspection) that a note was obtain from the parent of Child #5 stating that these medical records will be forth coming.

Plan of Correction: It is expected that this documentation shall be obtain this week.

Standard #: 63.2-1720.1-B-2
Description: Based on review, 1 of 2 staff files indicated that one staff person did not submit to fingerprinting and provide personal descriptive information described in subdivision B 2 of ? 19.2-392.02 prior to employment.

Evidence -
1. On the date of inspection, a fingerprint check was not available for review for Staff #2.
2. The start date for Staff #2 was listed as 02/22/2021.
3. Staff #2 was on site providing care on the date of inspection.

Plan of Correction: Staff #2 shall obtain a fingerprint check and obtain a finding for the FDH.
