Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 3/25/2021. The provider was contacted by telephone to initiate the inspection. There were (11) children present and (3) caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) caregiver records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-120-B
Description: Based on record review, the required records were not kept for each staff person.
Evidence:
1. Caregiver #2 was hired on 1/04/2021 and the provider did not have documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: I will call the references again tonight and right down what they say.

Standard #: 22VAC40-111-210-A
Description: Based on interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Caregiver #1 stated that the required 16 hours of training had not been completed for 2020.

Plan of Correction: I will make sure I receive a certificates when I complete the trainings.

Standard #: 22VAC40-191-60-C-2
Description: Based on interview, the provider did not obtain a central registry findings within 30 days of employment for a caregiver.
Evidence:
1. Caregiver #2 began employment on 1/04/2021 and the provider stated the central registry had not been sent and did not have the findings.

Plan of Correction: I will send it out this week.

Standard #: 63.2-1720.1-B-4
Description: Based on interview, the provider did not obtain results of the a Sex Offender registry check for each state an individual has resided in the past 5 years prior to the 1st day of employment for a caregiver and did not send for the Out of State Child Abuse and neglect registry from any state in which the individual has resided in the past five years by the end of the 30th day of employment.
Evidence:
1. The provider stated the Sex Offender registry for Maryland and the Out of State Child Abuse and Neglect Registry for Maryland had not been completed. Caregiver #2 lived in Maryland in the last 5 years and the date of hire was 1/04/2021.

Plan of Correction: I will send it out this week.
