Technical Assistance:

Licensing inspector discussed requirements for proof of identification with the provider.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 03/02/2021 and concluded on 03/03/2021. The provider was contacted by telephone to initiate the inspection. There were 10 children present and 3 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 caregivers records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct. Evidence: The records of Child A and Child B were last reviewed 01/2020.
Plan of Correction:	I have asked parents to review the information on file is still correct and sign.
Standard #:	22VAC40-111-90-A
Description:	Based on record review, the provider did not obtain documentation that a child has been adequately immunized. Evidence: Child B did not have an immunization record on file.
Plan of Correction:	I have forwarded proof of records.
