Standard #: 22VAC40-191-40-D-4-a Description: Based on record review, a household member did not have documentation of a completed Central Registry Findings.
Evidence:
1. Central Registry Findings are good for 5 years. Household Member #1's record contained an expired Central Registry Finding. The Central Registry Finding was dated 03/224/2016.Plan of Correction: The Household Member will complete a Central Registry Search and mail it. Once the findings are returned, the provider will send a copy of the results to the licensing inspector.

The Central Registry finding was dated 03/24/2016.Plan of Correction: Provider will complete a new Central Registry Search and send the licensing inspector a copy of the findings when they come back.
