Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on April 12, 2021 and concluded on April 13, 2021. The provider was contacted by video call to initiate the inspection. There were 11 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 caregiver/household member records submitted by the facility to ensure documentation was complete. CHOOSE ONE Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on review of one household member record, the required subsequent tuberculosis (TB) screening documentation is not in the record. Evidence: The record for household member #1 does not contain current documentation of a TB screening. Documentation in record is expired.
Plan of Correction:	I will make sure he goes tomorrow.
Standard #:	22VAC40-191-40-D-1-c
Description:	Based on review of one caregiver record, there is no documentation of subsequent central registry background check in record. Evidence: The record for caregiver #1 does not contain documentation of a current central registry background check. The documentation in the record is expired.
