Technical Assistance:
Discussed areas found in non-compliance but were not cited during this inspection as they are included in the "State of Emergency Suspension of DSS Licensing Child Care Regulation Requirements". The provider must still comply with all requirements once the emergency has passed: annual training hours, CPR/First Aid certification and tuberculosis screening. The LI discussed with the provider the documentation needed to submit a complete renewal application.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A /renewal inspection was initiated on 04/14/2021 and concluded on 04/19/2021. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 1 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on documentation review and interview with the provider, emergency evacuation procedures were not practiced monthly.
Evidence:
1) The emergency evacuation drills log did not have an emergency drill documented for the month of December 2020. The provider confirmed that they did not complete an emergency evacuation drill on that month.

Plan of Correction: The provider shall ensure that emergency evacuation drills are practiced monthly.

Standard #: 22VAC40-111-100-A
Description: Based on record review and interview with the provider, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The records for Child #3 (start date 09/03/2019) did not contain documentation of a physical examination. The provider confirmed that the physical examination was not in the record.

Plan of Correction: The provider shall request the physical examination documentation for Child #3.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview with the provider, the provider did not obtain documentation that the child has been adequately immunized before the child attended the family day home.
Evidence:
1) The record for Child #3 (start date 09/03/2019) did not contain documentation of the child being adequately immunized. The provider confirmed that there is no immunization record in Child #3's record.

Plan of Correction: The provider shall request the immunization documentation for Child #3.
