Technical Assistance:

The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 4/12/2021. Once the background check is received by the center, the results must be forwarded to the licensing inspector. A license renewal will not be issued prior to viewing satisfactory results of the outstanding central registry check.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was conducted on 4/9/2021. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. Description:	Based on observation and provider statement, the provider did not maintain an up-to date record for each child in care. Evidence: Child 1 did not have a record on the date of inspection. A vaccination record was the only documentation that the provider had available.
