Technical Assistance:

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs. Reminder: Background checks must be completed every 5 years.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. Ad monitoring inspection was initiated and concluded on April 15th, 2021. The provider was contacted by telephone to initiate the inspections. There were 2 children present with 1 caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 1 staff and 1 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #:	22VAC40-111-100-A
Description:	Based on a review of children's records, the provider failed to obtain documentation of physical examination by the 30th day of the child's attendance. Evidence: Child #2's record does not contain documentation of a physical examination by the 30th day of attendance. The child's first day of attendance was 1/14/2021 and the physical examination was documented 3/15/2021
Plan of Correction:	I will obtain documentation of children's physical examinations by the 30th day of attendance.
Standard #:	22VAC40-111-60-A
Description:	Based on a review of children's records, the provider failed to obtain documentation of all required information for each child enrolled. Evidence: Child #1 and Child #2's records do not contain documentation of the name, address and telephone number of two designated persons to contact in case of an emergency if the parents can not be reached;
Plan of Correction:	I will request the information from the parents and place in each child's record.
Standard #:	22VAC40-111-90-A
Description:	Based on a review of children's records, the provider failed to obtain documentation of immunizations by the first day of attendance. Evidence: Child #2's record does not contain documentation of immunizations on the first day of attendance. The child's first day of attendance was 1/14/2020, immunization document is dated 3/15/2021.
Plan of Correction:	I will obtain immunization records of children on the first day of attendance.
