Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/13/2021 and concluded on 4/13/2021. The provider was contacted by telephone to initiate the inspection. There were 9 children present and 3 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance.

Evidence:
Child#2's record contains the family day home's Liability Insurance Declaration form and the Information for Parents form; however, the forms have not been signed by the parents.

Plan of Correction: The forms have been signed.

Standard #: 63.2-1720.1-A
Description: Based on record review and interview, the provider did not obtain a repeat Central Registry search every five years for each staff member as required.

Evidence:
Staff#1's record does not contain a 5 year repeat Central Registry check. The Central Registry search on file expired on 3/4/2021.

Plan of Correction: The background check has been requested. The results will be submitted to the Licensing Inspector as soon as they arrive.

Standard #: 63.2-1721.1-A
Description: Based on record review, there is no documentation that the applicant initiated a repeat Central Registry search every five years as required.

Evidence:
Applicant#1 did not obtain a 5 year repeat central registry check. The Central Registry search on file expired on 3/14/2021.

Plan of Correction: The background check has been requested. The results will be submitted to the Licensing Inspector as soon as they arrive.
