This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 04/27/2021 and concluded on 04/30/2021. The provider was contacted by telephone to initiate the inspection. There was 1 child present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 1 child?s record and 1 staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-111-800-B Description: Based on documentation review and interview with the provider, the provider did not review the emergency plan annually.
Evidence:
1) On the date of inspection, the provider stated that she had not reviewed the Emergency Preparedness Plan (EPP). 2) Documentation of a review to the EPP could not be located.Plan of Correction: The provider shall review the emergency plan and document such review and any updates.
Standard #: 22VAC40-111-70-A Description: Based on record review and interview with the provider, parents were not provided in writing with all required information before the child's first day of attendance.
Evidence:
1) The record for Child #1 (start date 07/13/2020) did not contain the Policies for the administration of medications. 2) The provider could not locate the Policies for the administration of medications form in the record for Child #1.Plan of Correction: The provider shall contact the parents to provide the required information and request their signatures.
