Background check requirements for household members reaching the age of 14 years were discussed.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on 5/5/21. The provider was contacted by telephone to initiate the inspection. There were 5 children (11 points) and 2 caregivers present. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 caregiver's and 1 pet's record submitted by the facility to ensure documentation was complete. A virtual walk through of the day care area was conducted to assess the indoor space used for the children. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Description:	Based on records review, the provider did not review emergency contact information for a child's record annually. Evidence: The emergency contact information for Child # 1's record was last reviewed on 11/1/18.
Plan of Correction:	The child was part time and returned as full time during the pandemic. The information will be updated and it will be documented.
Standard #:	22VAC40-111-180-A
Description:	Based on record review and staff interview, the provider did not obtain results of a subsequent tuberculosis (TB) screening for a caregiver, every two years. Evidence: 1. Caregiver # 1's most recent TB screening expired on 3/22/21. 2. The provider stated that Caregiver # 1 has not obtained an updated screening.
Plan of Correction:	A current TB screening will be obtained from the caregiver and kept on file.
