Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was conducted on 5/14/2021. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and 4 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 4pm on Monday 5/24/2021. Standard #:	22VAC40-111-180-A
Description:	Based on record review, the provider did not obtain an updated tuberculosis (TB) screening or test for all staff and household members completed at least every 2 years. Evidence: Staff #1's last TB test was completed 5/8/2018. Adult Household Member #1's last TB test was completed 12/12/2018. Adult Household Member #2's last TB test was completed 5/8/2018.
Plan of Correction:	He's busy taking his finals. As soon as he will be done I will take him for TB testing.
Standard #:	22VAC40-191-40-D-4-a
Description:	Based on record review, the provider did not ensure that all adult household members have a completed Sworn Disclosure Statement on file. Evidence: Adult Household Member #3 does not have a Sworn Disclosure Statement on file.
Plan of Correction:	I will send you by the end of this week.
Standard #:	63.2-1721.1-B-2
Description:	Based on record review, the provider did not ensure that all adult household members submit to a fingerprint based criminal history background check. Evidence: Adult Household Member #3 does not have fingerprinting results on file.
