Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/18/2021 and was conducted on 05/27/2021. The provider was contacted by telephone to initiate the inspection. There were 4 children, totaling 9 points, and 1 provider present during the inspection. The inspector reviewed 2 children's records, 2 caregiver records, 1 pet record, and 1 household member record during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observations and discussion, the provider failed to maintain a working battery-operated weather band radio and extra batteries shall be kept in a designated area and available to caregivers at all times.

Evidence: The battery-operated weather band radio did not work.

Plan of Correction: The batteries will be replaced.

Standard #: 22VAC40-111-800-B
Description: Based on document review and discussion, the provider failed to review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

Evidence: The most recent documented review/update of the emergency plan was done in 04/2020; a 2021 review/update was not completed.

Plan of Correction: It will be reviewed today.

Standard #: 22VAC40-111-60-B
Description: Based on record review and discussion, the provider failed to ensure that each child's record contains all of the elements required by the standards.

Evidence:
1. The record for Child #1 did not contain the following information: the name, address and phone number for each custodial parent's place of employment (there was a place of business listed however a business phone number was not provided), the immunization records for the child as required by 22VAC40-111-90 and the results of the health examination for the child as required by 22VAC40-111-100.
2. The record for Child #2 did not contain the following information: the policy number of the child's medical insurance (a medical insurance provider was listed), the names and addresses of previously attended child day care and schools, and the immunization records for the child as required by 22VAC40-111-100.

Plan of Correction: Will get the information from the parents.

Standard #: 22VAC40-111-280
Description: Based on observation and discussion, the provider failed to ensure that potentially poisonous substances, such as but not limited to, cleaning agents, disinfectants and deodorizers, shall be stored away from food in areas inaccessible to children.

Evidence: A container of disinfecting wipes and a can of disinfectant spray were observed beside the bathroom sink during the virtual inspection. The disinfectant supplies were accessible to children.

Plan of Correction: They will be put in a cabinet.
